Creating Embed Forms

Professional FeaturesUpdated Sep 22, 20262 min read

Overview

Embed forms let you collect Human Design chart data from visitors on your website. Lead capture depends on the form settings and requires an email address. A form submission does not by itself confirm that a CRM lead was saved.

Creating a New Embed Form

  1. Open /workspace/forms in the web workspace to create or manage an embed form.
  2. Click Create New Form.
  3. Configure the form settings (name, fields, styling).
  4. Save the form.

Getting the Embed Code

After creating a form, click Get Embed Code to see options for embedding on your website. The Embed Code Generator supports three formats:

HTML (Static Sites)

Copy the HTML snippet and paste it into your website’s code. The HTML snippet uses an external script, so the host page must permit that script to run.

React (React Apps)

Copy the React component code to integrate the form into a React application.

WordPress (Shortcode)

The WordPress shortcode requires a compatible shortcode handler, such as the HDAI Embed Forms plugin. Pasting the shortcode alone is not sufficient.

Additional Options

  • Direct Link – Get a standalone URL for the form that you can share directly.
  • Custom Height – Configure the form height to match your website’s layout.
  • Copy to Clipboard – One-click copy for any embed format.

What Happens When Someone Fills Out the Form

  1. The visitor completes the fields required by the selected form type.
  2. Chart and report-order forms request chart generation. Email-list and contact forms do not generate a chart.
  3. A CRM lead can be created when lead capture is enabled and an email address is supplied; chart data is only relevant to chart-generating form types.
  4. A notification is attempted after a lead is created. Email notifications also depend on the form’s notification setting and an available recipient email address. A successful submission does not guarantee notification delivery.
  5. A webhook request is attempted when a webhook URL is configured and webhooks are enabled. A successful form submission does not confirm webhook delivery.
